Authorized and Accepted: You are hereby authorized to make the necessary specified repairs. I understand that payment in full will be due upon release of the vehicle. Including additional supplemental damage charges. I hereby grant you and your employees permission to operate the vehicle on streets, highways, or elsewhere for the sole purpose of testing and/or inspection. An express mechanic's lien is hereby acknowledged on the specific vehicle to secure the amount of repairs. This repair facility will not be help responsible for loss or damage to vehicle or articles left in vehicle, in any case that is beyond our control. Old parts removed from vehicle will be destroyed unless otherwise instructed.

I authorize all final payments to be made payable directly to this facility. I am also aware that should an insurance check be sent directly to me, that I am obligated to forward to this repair facility with my endorsement. I authorize this repair facility to act as power of attorney to sign payments on my behalf should they be sent directly to this repair facility.
